PHASE ONE
0-6 WEEKS
RANGE OF MOTION
0-2 weeks: None
2-4 weeks: Begin PROM
Limit 140° flexion | 45° ER | 20° extension | 45° abduction | NO IR.
IMMOBILIZER
0-4 weeks: Immobilized at all times, day and night, with abductor pillow
4-6 weeks: Immobilized at all times, day and night, with no abductor pillow.
EXERCISES
Elbow and wrist ROM, grip strengthening and pendulums.
Begin PROM
Colman’s scapular stabilization.
PHASE TWO
6-12 WEEKS
RANGE OF MOTION
Progress PROM to full. Start active assist and progress to AROM.
IMMOBILIZER
None
EXERCISES
Continue Phase One
Begin active-assisted activities, exercises, begin passive IR.
Continue scapular program. Begin wall climbs, ladder and wand exercise home program.
PHASE THREE
12-16 WEEKS
RANGE OF MOTION
Gradual return to full AROM
IMMOBILIZER
None
EXERCISES
Begin resistive strengthening in all planes.
Advance scapular program.
Cycling, running and moving as tolerated at 12 weeks.
PHASE FOUR
4-6 MONTHS***
RANGE OF MOTION
Full and pain-free
IMMOBILIZER
None
EXERCISES
Continue with strengthening and stabilization exercises.
Begin plyometric, throwing and racquet program, continue with endurance activities.
Maintain ROM and flexibility. (ROM = range of motion)
6 months: Return to sports activities after clearance from physician.
